Where does the weight-loss data come from?
Each drug's expected weight loss is modeled from its FDA phase 3 trial primary endpoint. Wegovy uses STEP-1 (14.9% at 68 weeks), Zepbound uses SURMOUNT-1 (22.5% at 72 weeks), Mounjaro uses SURPASS, oral Wegovy uses OASIS-4, and Foundayo (orforglipron) uses ATTAIN-1. We model the curve as an exponential approach to plateau, which closely matches published trial data.
Where do the prices come from?
Manufacturer wholesale (WAC) prices come from Novo Nordisk and Eli Lilly public pricing. Self-pay manufacturer prices come from LillyDirect and NovoCare. Telehealth prices come from public pricing pages at Hims, Ro, Found, Lemonaid, and LifeMD as of April 2026. Insurance copays are typical ranges; your actual copay depends on your specific plan.
Why is the cost-per-pound number useful?
It lets you compare drugs apples-to-apples on cost-efficiency. A drug that costs more per month but produces dramatically more weight loss may actually be cheaper per pound lost. Use the comparison table to find your best option.
What about side effects, contraindications, and dose escalation?
This calculator focuses on cost and efficacy. It does not model side effects, drug interactions, dose escalation schedules, or individual response variation. These are critical factors and require a licensed prescriber. We strongly recommend consulting a doctor or telehealth NP/RD before starting any GLP-1.
Are compounded GLP-1s still legal?
FDA cracked down on compounded semaglutide in 2025 after Novo Nordisk’s shortage was resolved. Compounded tirzepatide remains in a gray area. Telehealth platforms like Hims and Ro have shifted toward branded drugs and lower-dose self-pay vials. The "telehealth-compounded" option in this calculator may not be available for all drugs.
Which GLP-1 has the highest weight loss?
Among FDA-approved drugs, Zepbound (tirzepatide) has the highest in clinical trials at 22.5% mean weight loss in SURMOUNT-1. Mounjaro (same molecule) shows similar off-label results. Pending drugs like CagriSema (20.4%) and retatrutide (28.7% in phase 3) may exceed this once approved.
Is GLP-1 covered by insurance?
It varies. Drugs FDA-approved for type 2 diabetes (Ozempic, Mounjaro) are widely covered for diabetic patients. Drugs FDA-approved for weight loss (Wegovy, Zepbound, Saxenda) face more denials—many plans require BMI ≥30 plus a comorbidity, and some exclude weight-loss drugs entirely. Medicare does not cover drugs prescribed solely for weight loss.
